THE PRESENTATION: The Columbia Accident Investigation Board (CIAB) determined that the physical root cause of the Space Shuttle Columbia accident was a breach in the Thermal Protection System on the leading edge of the left wing. The findings and recommendations of the CIAB covered areas that included the root cause as well as human causes such as training and scheduling, and latent causes such as organization. This presentation will cover these areas and make a comparison of this accident to another accident that was equally shocking to the world, the sinking of the Titanic.

Speaker Biography: Dr. Daniel P. Dennies is a Technical Fellow for the Boeing Company and has 29 years experience as a metallurgist and performing failure investigations.  He authored the book “How to Organize and Run a Failure Investigation,” (see local course announcement next page) released in September 2005.   The majority of his career has been in the aerospace industry working on projects such as the Space Shuttle Main Engine, the National Launch System, the National Aerospace Plane, expendable launch systems like Delta and Titan, B1, C-17, the International Space Station, and, most recently, proprietary projects.

The West Michigan Chapter of ASM International is hosting a two day course by Dr. Daniel Dennies of the Boeing Corp. Dr. Dennies is serving his third year as an ASMI Trustee, and has been involved in some well known aerospace failure investigations. The course dates are March 17 and 18 (TUESDAY AND WEDNESDAY). This course is for anyone who NEEDS to PROCURE or SPECIFY or PARTICIPATE in any failure investigation. It is not a technical metallurgy course. It is on problem definition and solving methods and is a good introduction for any type of failure investigation, including MANUFACTURING PROCESS PROBLEMS.

ASM Leadership Days 2008

ASM Leadership Days is an annual, weekend conference sponsored by ASM to support the leadership of the individual chapters. There are workshops, speakers, idea exchanges, social activities, etc. to encourage information exchange among the attending chapters.

This year's Leadership Days conference was held in Cleveland and Metals Park, Ohio in early August. The West Michigan chapter was represented by Brian Hall, Supplier Quality Manager of Bronson Precision Products, Bronson, MI, this year.

On behalf of the West Michigan chapter, Brian accepted the 5 Star Chapter award from ASM President Dianne Chong. ( See attached photo).

If you are interested in participating in the leadership of the West Michigan Chapter, we are always looking for new people to get involved. Activities can take on any range of involvement from brainstorming, to planning meetings or events, to participating in conferences, like ASM Leadership Days. Thomas Tom, WMASM Mini-Grant Program coordinator presents award certificate and check to teacher, Patrick Rabbitt, Mona Shores Schools.

The 2008 West Michigan ASM International Mini-Grant awards have been selected. This year two recipients are receiving money from WMASM to support their school system's curriculum to encourage students in the field of material sciences.

Patrick Rabbitt, a previous award recipient, was awarded $1200, which will cover one third the cost of a Smithy Granite mill/lathe for the Advanced Metals Course at Mona Shores School District, in Muskegon. WMASM member, Debbie Aliya will make a classroom presentation to the students, in the fall, on how mechanical properties and microstructure affect machinability. The award was presented at the May meeting of the Mona Shores School Board. The Board expressed their appreciation to WMASM for their support of the student materials science program.

Gordon DeKoekkoek, of Grand Rapids Christian Schools, a first time award recipient, was awarded $426 to cover supplies needed for the Welding Module. In the fall, WMASM member, Debbie Aliya will make a classroom presentation to the students, to address maintaining excellence in welding.

WMASM appreciates the opportunity to encourage students to pursue careers in materials sciences. We look forward to more opportunities for that in 2009.


The quality of our chapter and the resources it offers depends on you. By getting involved in your chapter, you help define its direction and determine the types of program and activities that are made available to you and your colleagues.

We have made great progress through a group of volunteers who have been very active. However the only way to make our chapter better is if more members become actively involved. Participation in leadership of the chapter only requires attending monthly executive committee meetings and contributing 1 - 2 hours per month between meetings.
